Edmonds School District Launches Final Forms for Athletics Eligibility
In order to be cleared for Fall Sports you will need:
1) Complete Final Forms (see below)
2) Current Sports Physical turned into final forms or emailed to Emily at crumsmithe936@edmonds.wednet.edu (they are good for 2 years)
The Edmonds School District is excited to announce a new partnership with FinalForms, an online platform designed to streamline the completion and management of athletic forms and data. Beginning with this year’s high school summer activities, families will use FinalForms to complete and sign athletic participation paperwork quickly, securely, and efficiently.
To access your account, visit the FinalForms website and click on the “Click here for a confirmation email” link under the Parent Icon. Then enter your parent email that is on file with your student’s Skyward account. FinalForms will send you an email for you to complete your account registration. If you need additional support, please read through the FinalForms Registration instructions website.
